The patient is a 69 year old female with recurrent decay of the remaining maxillary dentition. The patient knew her remaining teeth needed to be extracted but she did not want to wear dentures for the rest of her life.
The prosthetic treatment plan was to restore the maxillary teeth as traditional porcelain bridgework supported by dental implants.
We fabricated an immediate denture and surgical guide for the placement of eight maxillary implants to be restored as a series of four, three unit bridges.
The surgical care was provided by Dr. Michelle Cabret-Carlotti at Desert Palm Surgical Group.

We used duplicate milled abutments to minimize treatment time and provide predictable results.
Using duplicate milled abutments on multiple unit cases minimizes treatment time. The original abutment is placed, torqued and closed while its twin is sent to the lab for fabrication of the final restorations. This eliminates the need to place and remove multiple abutments during the restorative phase of treatment. Provisional restorations are placed at the time of the abutment placement. This allows the patient time to preview and approve the smile and occlusion before the final restorations are completed. The patient can evaluate the size, shape and color of the teeth during this period.
This protocol reduces the number and length of appointments needed to restore multiple unit cases.
We use a corrected cast final impression for multiple unit milled abutment cases. This assures the final impression is accurate eliminating the need for metal try-ins. The combination of the duplicate milled abutments and accurate master cast creates a level of accuracy and predictability previously unavailable. Utilizing the new technology reduces treatment time, improves quality and assures a predictable result.
